A back injury that keeps an employee out of work for months while he renovates his house. A slip-and-fall in a store that was rehearsed twice before the camera-free aisle was found. A disability claim filed by someone running a landscaping crew on weekends. FSO Guard’s licensed investigators provide insurance fraud and workers’ compensation investigations in Miami and South Florida for employers, carriers, administrators and attorneys — lawful, discreet, and documented to the standard a Judge of Compensation Claims expects.
Red flags that justify an investigation
- Injury reported on a Monday morning, right after a holiday, or immediately after a layoff notice, discipline or a denied vacation.
- No witnesses, or witnesses whose accounts change.
- Claimant is hard to reach, refuses light duty, or has a history of prior claims across employers.
- Medical treatment inconsistent with the injury, or providers known for volume claims.
- Reports of the claimant working elsewhere, exercising, traveling or posting activity online.
- A liability claim at a location with no cameras, filed weeks later, with an attorney already retained.
None of these proves fraud. They justify verification — which protects honest claimants as much as it exposes dishonest ones.

What we do
Activity surveillance
Planned observation of the claimant in public — leaving home, lifting, driving, working, exercising, attending events — with time-stamped, geotagged video from unmarked vehicles. Two investigators when the claimant is mobile. Reports describe exactly what was observed, without editorializing, so the video speaks for itself.
Claimant background and prior claims
Identity verification, prior workers’ compensation and liability claims, litigation history, other employment, business ownership and licenses, and criminal history where relevant — all from lawful sources with a documented permissible purpose.
Employment and activity verification
Is the claimant working for another employer or running a business while collecting benefits? We verify through records, field checks and lawful observation.
Social media and open-source review
Lawful review and preservation of publicly available posts, photos, marketplace listings and check-ins that contradict the claimed limitations — captured properly so they are admissible.
Scene and liability investigations
For premises and auto liability: scene documentation and measurements, camera canvass, witness identification and recorded statements, weather and lighting conditions, prior incident history at the location.
Witness canvass and interviews
Locating co-workers, neighbors and bystanders, taking recorded or written statements, and delivering them in a format counsel can use.
How the investigation is run
- Referral and intake — claim summary, claimant profile, restrictions, deposition or IME dates, budget.
- Plan — surveillance days chosen around the claimant’s routine and key dates; background work first, because it is cheap and often decisive.
- Field work — surveillance, canvass, verification. Daily updates to the adjuster or attorney.
- Report — chronological narrative, still frames and video, records and statements as exhibits, delivered securely. Investigators are available for depositions and hearings.
Rules we follow
We observe only in public, never trespass, never record private conversations without consent, never contact claimants under false pretenses, never place trackers on vehicles we have no right to track, and never obtain protected medical or financial records without lawful authority. Investigations are conducted under a written agreement with a documented lawful purpose. This is not caution for its own sake — evidence obtained any other way is excluded, and the employer or carrier becomes the defendant.
